	Based on Article 6 and 9 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC, I declare that I am expressing consent to the processing of my personal data by ‘ETP OFFSHORE’ with headquarters in w Gdynia, ul. Kielecka 2. 81-304 Gdynia, registered under KRS number 0000517449 with the NIP number 5862291274 and REGON 222126775, which will be their administrator, for current and future activities to mediate in the recruitment and employment process for foreign shipowners, including for the purposes necessary to implement the seafarer employment contract and its financial security, in accordance with the requirements of the MLC 2006 Convention. I declare that I consent to the transfer of personal data to third countries, for which the European Commission has not issued a decision stating an appropriate level of protection pursuant to GDPR art. 45,3. I declare that I am expressing consent to the processing of my personal data for these purposes voluntarily and I have been informed about the possibility of withdrawing consent at any time.
	Na podstawie art. 6 i 9 Rozporządzenia Parlamentu Europejskiego i Rady (UE) 2016/679 z dnia 27 kwietnia 2016 r. w sprawie ochrony osób fizycznych w związku z przetwarzaniem danych osobowych i w sprawie swobodnego przepływu takich danych oraz uchylenia dyrektywy 95/46/WE oświadczam, że wyrażam zgodę na przetwarzanie moich danych osobowych przez ‘ETP OFFSHORE’ z siedzibą w Gdyni, ul. ‘Kielecka 2. 81-304 Gdynia’, zarejestrowaną pod numerem KRS 0000517449 posiadającą numer NIP 5862291274 oraz REGON 222126775, która będzie ich administratorem, dla celów obecnych i przyszłych działań w celu pośrednictwa w procesie rekrutacji i zatrudnienia u armatora zagranicznego, w tym do celów niezbędnych do realizacji marynarskiej umowy o pracę i jej finansowego zabezpieczenia, zgodnie z wymaganiami Konwencji MLC 2006. Oświadczam, że wyrażam zgodę na przesyłanie danych osobowych do państw trzecich, co do których Komisja Europejska nie wydała decyzji stwierdzającej odpowiedni stopień ochrony na mocy na mocy GDPR art.45, 3.
